Licensee Login Dentists and hygienists may now login at any time to make changes to your information on file with the Board. Your login ID is your Louisiana license number, and your default password is the last four digits of your social security number. If you would like to change your password, you may do so from the licensee information screen. If you forget your password, you must have a current email address on file with the Board in order to have your password sent to you. If you do not know your Louisiana license number, you may look it up here. Your License ID is your license number and your (initial) password is the last 4 digits of your SSN. You may change your password once you are logged in. The Board no longer mails license renewal certificates. You may print your current license renewal certificate at any time by logging in below and selecting "certificate." Office permits will be found under "office permits." Your personal anesthesia permit is printed on your license renewal certificate.
